Service Registration for December 2023

PLEASE READ
By attending services or events at AJ, all attendees confirm that:
  • You have not tested positive for Covid-19 in the past 5 days.
  • If you tested positive for Covid-19 in the past 6 to 10 days, you must wear a high-filtration (N95/KN95) mask inside the building through day 10.
  • If you were exposed to Covid-19, you don't have symptoms, and will remain masked for 5 days while in the AJ building.
  • You do not currently have symptoms consistent with Covid-19 including but not limited to fever, cough, shortness of breath, loss of taste or smell.
Luncheon Registration
For the Shabbat Luncheon on Saturday, December 16, please use the Rav Shai 5th Anniversary Celebration form.
